Special Programs

Falls Management Program

Falls are a common and critical problem faced by acute care hospitals, nursing homes, assisted living facilities, and Home Health care patients.

Fall Prevention Saves Lives and Medicare Dollars

  • Acute Care Facilities: up to 20% of patients in acute care facilities experience a fall.

  • Long-Term Care Facilities: As many as 50% of patients in LTC facilities experience a fall.

  • As many as one out of 3 adults age 65 years or older fall each year. Many never admit their falls to healthcare providers.

We perform a fall risk assessment upon admission and upon significant events to determine the level of fall risk. At Chambers Home Health our plan of care is developed towards safety and fall prevention.
