Falls are a common and critical problem faced by acute care hospitals, nursing homes, assisted living facilities, and Home Health care patients.
Fall Prevention Saves Lives and Medicare Dollars
- Acute Care Facilities: up to 20% of patients in acute care facilities experience a fall.
- Long-Term Care Facilities: As many as 50% of patients in LTC facilities experience a fall.
- As many as one out of 3 adults age 65 years or older fall each year. Many never admit their falls to healthcare providers.
We perform a fall risk assessment upon admission and upon significant events to determine the level of fall risk. At Chambers Home Health our plan of care is developed towards safety and fall prevention.

Our Bridge Program focuses on pain and symptom control for patients under our Home Health care.
Management is directed towards providing comfort care in the home setting to patients who may or may not be receiving treatment such as chemotherapy, radiation, dialysis, etc
Our staff is trained in addressing issues related to chronic or debilitating illnesses. Our team includes nursing, social work, spiritual, and Home Health aide services as well as therapy when indicated. The goal is to prevent repeated trips to the ER and/or hospital readmissions. Our goal is to provide quality of life for both the patient and the family. The care is focused on providing relief from the symptoms and stress of a serious illness.
